The "Fine Print" (Member Agreement Highlights) Commitment: All memberships require a 3-month minimum initial term to ensure clinical results.
Cancellation Policy: After the 3-month term, we require 30 days' written notice to cancel. Any payments scheduled within that 30-day window will process as usual.
Premium Protection (Tiers 2 & 3): Laser and Medical Weight Management slots are high-demand. Cancellations with less than 48 hours' notice will incur a $100 "Room Fee" or forfeiture of that month's credit.
Medical Eligibility (Tier 3): Sirius Elite membership is subject to medical clearance for GLP-1 therapy. If a member is no longer a candidate, they will be transitioned to the Nova Corrective tier ($399) automatically.
Non-Transferable: Credits are for the member's use only.
Tirzepatide Clause (Tier 3 Only):
Medication Surcharge: The Sirius Elite pricing covers Semaglutide therapy. If a patient is prescribed Tirzepatide due to medical necessity or preference, a +$200/month surcharge will apply to cover the higher medication cost.
Dosage Caps: Membership covers medication up to 4ml. Dosages exceeding this cap may require a small additional supply fee.
